E911 Physical Address Change Request

For each phone line and telephone number used with Digium Cloud Services’ service, the customer must register with Digium Cloud Services the physical location (including floor and suite number) where customer will be using the Digium Cloud Services’ service with that phone number.  Customer's initial location will be registered as part of subscribing to the service. However, it is incumbent on customer to update customer's physical location using this form or by calling 888-305-3850 or by emailing [email protected] if customer's address changes. Customers who are residential users or who have nomadic equipment or phones where the calling location changes as the device moves to another location may not use Digium Cloud Services’ service.  If customer is a residential user or nomadic user who attempts to make unauthorized use of Digium Cloud Services' service from the new location, 911 calls made will be sent to an emergency center near your old address.
